Over-the-Counter

Under the supervision of our clinic physician, Duchesne Academy will provide some over-the counter medications in our clinic. Medication will be administered based on permission granted by the Over-the-Counter Medication form, located in the Magnus Health Portal, as well as by the guidelines outlined in our clinic protocol. Parents of students in PK-8 will be contacted prior to OTC administration to avoid any overlapping of medications.

Prescription

For daily prescription medication that needs to be given during the school day, a parent/guardian must complete a Daily Medication Form on the Magnus Health Portal, and the medication must come with the current prescription intact.
