Any student seeking an accommodations plan at Duchesne must provide comprehensive, up-to-date documentation that meets the school’s criteria and that has been conducted by an approved, appropriately credentialed evaluator.  

Since eligibility for accommodations is contingent upon adherence to Duchesne’s guidelines and parent partnership with the school, parents must speak with the appropriate Learning Specialist before initiating a new or updated evaluation. 

The Duchesne Learning Specialists will provide evaluation criteria and a list of approved evaluators upon request. 

Duchesne maintains the right to determine whether to accept an evaluation from an unapproved evaluator. 

Evaluations are considered valid for three years, at which point a re-evaluation will be necessary.
