Please note that the required records release and teacher recommendation forms will be available for you to download and copy once you have filled in the grade level apply on the Student Information page of the online application. The grade level will trigger the correct forms' link at the bottom of the Student Information page as "Supplemental Forms".
Infosnap is the chosen method for submitting applications to Duchesne Academy because their innovative and secure technology protects families' information (see Security and Privacy). It is unique because users can enter their information once and direct that data to be reused for multiple forms at Duchesne, or for any other organizations that use Infosnap.
